An antique medical vaginal speculum from the 1920's made by Tridesta, designed for internal inspection at a time before less invasive methods were possible it is designed to expand after insertion with an adjustable locking mechanism to secure the degree of internal dilation. This example is 12cm long. The catalogue illustration was taken from the Down Brothers catalogue dated 1900, the example for sale here is a little later.
